Arkansas River Flooding Disaster Information

Delta Dental of Arkansas extends concern and sympathy to everyone whose lives have been impacted by the flooding of the Arkansas River. If you are a Delta Dental of Arkansas business client or an individual member in an affected area, please stay safe.

 

We’re ready to help with your dental and vision insurance over the coming months by providing extensions on premium payments, replacing lost or damaged ID cards or certificates of coverage, and customer service assistance to work with you on a case-by-case basis.

 

You have two options to request an extension (for up to 60 days starting May 21, 2019 and ending July 20, 2019) on paying your Delta Dental premiums.

  1. Contact the appropriate Delta Dental of Arkansas customer service center listed below. Our customer service centers are open Monday – Friday, 7 a,m, to 7 p.m., CST.  OR
  2. Complete and submit the appropriate form below.

Please note, that per State of Arkansas Insurance Department, all business clients and individual members requesting an extension of premium payment must:

  1. live in an area affected by the flood, and,
  2. have suffered significant property damage, injuries, or other loss of life as a result of the flood.
